A Clinical Review of 1292 cases of Burn Patients



Abstract
This is a review and clinical analysis of 1292 cases of burn patients which treated at the Department of surgery, Hang gan Sacred heart hospital, college of medicine, Hallym University during 3 years 6 months from July, 1986 to December, 1989.
The following results were obtained.
1) Age distribution in burn patients showed that the highest incidence of #age group was below 10 years old (471 cases, 36%).
2) Seasonal distribution showed that summer was the highest incidence but no significant seasonal difference was observed.
3) Flameburn was 49.9%, and 44.4% in scalding burn.
4) The most frequent period of interval between accident and arrival to hospital was within 24 hours (871 cases, 67.4%).
5) Most of patients (44.8%) were lesser affected than 20% extent of burn, 192 patients (14.9%) were in 21 to 30% extent of burn.
6) The most frequent duration of treatment was within l0 dyas (38.5%).
7) 510 cases (59.69%) showed hemoconcentration. Severely hemoconcentrated groups (Hct$gt;51%) showed very high mortality rate.
8) Skin graft was performed in 329 cases (25.5%) and the most frequent period of grafting was within 5 weeks (72%).
9) The main complication were wound infection (25.8%), G-U tract infection (4.6%), and pneumonia (4.5%) in order.
10) In 659 cases of burn wound culture, the main organism were Pseudomonas aeruginosa 360 (63.2%), Staphylococcus aureus 129 (21.5%), Acinetobacter calcoaceticus 25 (4.2%) and others.
11) The overall mortality rate was 19.3% (250/1292) and the most frequent cause of death was sepsis in 132 cases (52. 8%), followed by pulmonary complication ion in 100 cases (40. 0%), and burn shock in 13 cases (5.2%).
12) The highest mortality rate was in flame burn (31.6%), and the lowerest mortality rate was in scalding burn (1. 7%).
13) The mortality rate was closely related to the extent of burn areas. causes of burn, and patients age.
